Pediatric Trauma - Management of the Pediatric Trauma Patient
Needle Cricothyrotomy
1. Use Universal Precautions
2. Prepare equipment- 12 or 14 gauge catheter attached to a 3cc syringe with saline
3. Maintain in-line stabilization if c-spine injury is suspected
4. Locate the landmarks – thyroid cartilage, cricoid cartilage and cricothyroid membrane
5. Prep the area
6. Grasp the larynx with the thumb and middle finger
7. Insert the needle through the skin at a 30°caudal angle
8. Advance while aspirating through the cricothyroid membrane until air bubbles are visible
9. Connect ETCO2 and jet ventilator or bag-mask device
10. Ventilate the patient at a 1:3 inspiratory to expiratory ratio
11. Secure catheter
12. Reassess the patient frequently and transport to nearest appropriate facility
