Predisposition to Hypothermia in Trauma

  • Greater relative body surface area.
  • Less insulation and thinner skin.
  • Environmental factors.
  • Cold IV fluids.

Hypothermia in the setting of trauma can significantly increase mortality, especially in association with acidosis and coagulopathy. Children are predisposed to becoming hypothermic in trauma due to several factors. Amongst them are: a greater relative body surface that increases heat exchange with the environment; little insulating fatty tissue and a thin dermis that can lead to heat loss through the skin; and environmental factors such as being wet or being in a cold climate. Other factors that can be prevented include: leaving the patient exposed during the trauma assessment and use of cold IV fluids during resuscitation.
