An early systolic impulse with a presystolic component might be felt in a patient with coronary artery disease. The presystolic impulse is the palpable equivalent of an audible fourth heart sound. In the video, note the presystolic impulse occurs before the carotid pulse and the early systolic component occurs with the beginning of the carotid pulse.
