The combination of mitral regurgitation and mitral stenosis is almost always of rheumatic etiology. The classic apical murmur of mitral regurgitation is holosystolic and high frequency and that of mitral stenosis is diastolic and low frequency. With mitral stenosis, a loud first sound and opening snap are typical, and a left ventricular third sound is incompatible with the lesion.
